Query Build Basics

Once you've imported the query you will need to open it up and change the date parameter to the previous month (this will need to occur each time you run it) e.g. for your first data set change the parameters to bweteen 01/08/18 and 29/08/18 this should capture all the FBC results between this period. If you're finding there aren't 10 results then use another measure e.g. LFT (a new query will need to be created for this).

If this still isn't work try the query using the term "full blood count" instead of "complete blood count".

Spreadsheets

Put the date that the data was collected i.e. your first set of data is submitted by September 10th but the data itself relates to August so put an August date in the first column of the spreadsheet and refer to this data as your August data. It's important that the date is entered in a DD/MM/YY format.

When entering the data please don't add or delete rows. Only enter data for a maximum of 10 patients each month. If you have less than 10 patients for a given month leave those spaces blank and move on.

Governance

Once involved, PHOs will support their practices by:

  • Participate in planning and implementing the programme.
  • Support and provide facilitation at learning sessions.
  • Encouraging general practices to hold monthly facilitated practice-based meetings to reflect on their patient safety data and identify areas of change to be tested within the practice environment.
  • Supporting general practices by establishing an appropriate infrastructure to successfully implement and sustain the improvements made.
  • Taking part in an ongoing evaluation of the programme.
  • Coaching and support in quality improvement.
  • Develop, fund and run learning sessions.
  • Lead the development and testing of new change packages.
  • Evaluate outcomes of the Safety in Practice initiative to determine future direction and participation.

Learning Sessions

Learning sessions are vital to Safety in Practice. Quarterly, practices come to share challenges and successes, learning improvement skills and understand the programme in more depth. Learning sets last approximately three hours, are held in the evening at a convenient location and include a light dinner.

As learning sessions are vital to the programme it is important that a minimum of two representatives from a practice or pharmacy (the Patient Safety Champion team) attend learning sessions.

Safety in Practice has a 3-year curriculum, once you have completed these 3 years and the requirements within this you will be a fully accredited Safety in Practice clinic or pharmacy! You will be issued with a framed certificate acknowledging your hard work and improvement efforts. You will then transition into our Alumni Network, which will help you maintain your accreditation.

You will be sent an email with your logon details for the e-Learning module. If you do not receive this but other members of your team have please contact us as info@safetyinpractice.co.nz to request an account.

Alumni Network

The Alumni Network is an elite group of primary healthcare providers who are certified as completing the 3-year Safety in Practice curriculum. They are fully accredited Safety in Practice clinics or pharmacies.

The Alumni Network provides connections and support to maintain quality improvement initiatives after completing the core Safety in Practice programme.

As a part of the Safety in Practice Alumni Network you will be provided with on-going access to resources and some support. This is to help you sustain the improvements that you have already made, and identify new quality improvement opportunities in other areas. The network will also keep you connected with updates and news in the area of safety and quality improvement. Please note there is no financial reimbursement for involvement in the Alumni Network.

As part of the Alumni Network, you will

  • Have access to the SiP website and associated resources
  • Receive advice and support from the Project Team if required
  • Be subscribed to an Alumni Newsletter

As a result of completing the Safety in Practice curriculum you will have completed at least two of the three safety climate tools.  You are encouraged to continue to use the resources relating to these tools and apply them to your business as usual. If you wish to complete the safety climate survey, you may request this from the project team and the opportunity will be provided on a case-by-case basis.
